Lutheran Senior LIFE at Union Welcomes Its First PACE Participant

Lutheran Senior LIFE at Union is proud to celebrate a meaningful milestone: welcoming our first participant to the Program of All-Inclusive Care for the Elderly (PACE). This moment marks the beginning of a new chapter for our team and for older adults in Union County who want to remain healthy, independent, and connected to their communities.

Located in Plainfield, Lutheran Senior LIFE at Union provides individualized, team-based care designed around each participant’s medical, emotional, and social needs. Through coordinated health care, rehabilitation, meals, transportation, social engagement, and support at home, our PACE team helps participants continue living safely in the community.

Welcoming our first participant reflects the dedication, compassion, and collaboration of the entire Lutheran Senior LIFE at Union team. We are grateful to everyone who helped make this milestone possible and honored to begin building lasting relationships with participants, their families and our community partners.

We look forward to expanding access to the PACE model of care and supporting more Union County seniors as they continue to live with dignity and purpose in the homes and communities they know.
